you will use throughout the Study.<\/li>\n<li>If you received a User Name and Password, enter it, then click Sign In.<\/li>\n<li>If you received a One Time Code, click on that link and enter your code\n<ul>\n<li>You will then be prompted to create a User Name and Password<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/li>\n<li>[_] You can use your laptop or your smartphone.<\/li>\n<li>[_] If using your smartphone, download the Netsmart myHealthPointe app.<\/li>\n<li>[_] If you are using your laptop or desktop, \u00a0 <a href=\"https:\/\/myhpconsumer.netsmartcloud.com\/\">CLICK THIS LINK.<\/a>\u00a0 With your portal you can request appointments, send messages to your Care Team, get your personal health information, view medication, make refill requests, and more.\u00a0 You can also participate in the Brain Health Study.<\/li>\n<\/ol>\n<p>PLEASE NOTE:\u00a0 The Patient Portal is for non-emergency use only.\u00a0 If you are experiencing a medical emergency please dial 911 from your mobile phone or your Beatitudes phone.\u00a0 If you use your Beatitudes phone, Security will automatically be notified and will also respond.\u00a0 Security&#8217;s direct phone number is x16172 or 602-433-6172 on
